According to Article 13(1) and (2) of the Regulation (EU) 2016\/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95\/46\/EC (hereinafter referred to as “GDPR”; Official Journal of the European Union L 119 of 04.05.2016), I inform you that:<\/strong><\/p>\n

1. The administrator of your personal data is: Excelmed SC Monika R\u0105czkowiak Bartosz R\u0105czkowiak, located at 6\/2 Miko\u0142aja Kopernika Street, Szczecin, tel.: +48 91 422 11 00. Contact with the Data Protection Officer can be made via email: ido@exceldent.pl<\/a>.<\/p>\n

2.\u00a0 Your personal data will be processed for the purpose of providing medical services, based on Article 6(1)(c) and Article 9(2)(h) of the GDPR.<\/p>\n

3. Recipients of your personal data will only be entities authorized to obtain personal data under applicable law, as well as analytical laboratories and other entities, including medical entities, with which the Administrator has concluded agreements for the processing of data entrusted to provide medical services, administrative, IT, accounting, and related social security services.<\/p>\n

4. Your personal data will be stored in accordance with the applicable law, in particular the Act of 6 November 2008 on Patient Rights and the Patient Rights Ombudsman, for a period of 20 years. In the case of a patient’s death, data will be kept for 30 years.<\/p>\n

5. You have the right to request access to your personal data, their rectification, or restriction of processing from the administrator.<\/p>\n

6. You have the right to lodge a complaint with the President of the Office for Personal Data Protection.<\/p>\n

7.
